Police chiefs have apologized for years of 'deviation and denial' over the Hillsborough disaster and vowed to bring about cultural change to prevent a repeat of the failures.

Nearly 34 years after the crash that killed 97 soccer fans, they have outlined plans to formalize a "duty of candor" on officials and stop "false narratives" that seek to minimize liability.
